Official Contest Rules

Sweepstakes Time Period:

  • November 1, 2008 at 12:00:01 am CST and ends December 31, 2008 at 11:59:59 pm CST.

How to Enter the Sweepstakes:

  • One online entry per person through the survey. Unlimited entries per person by submitting referrals.
  • One in-person entry at the ASHP tradeshow booth.
  • Pharmacy OneSource is not responsible for and will not consider incomplete or incorrect entries as potential contest winners.

Eligibility:

  • NO PURCHASE NECESSARY. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING. Void where prohibited.
  • The sweepstakes is open to pharmacists.
  • You must be 18 years of age or older and a current resident of the U.S. to participate in the contest.
  • Full-time and part-time employees of Pharmacy OneSource, and any organization or agency affiliated with this contest, their immediate family or anyone who resides in the same household as the employee are ineligible to participate and/or win.
Sweepstakes Prizes:
  • Approximate value of Grand Prize is $3,000.
  • Pharmacy OneSource will randomly select one potential sweepstakes winner from all eligible entries during the Promotion Period. The potential winner will be notified by e-mail or phone. In the event that a potential winner of the prize is disqualified for any reason, Pharmacy OneSource will award the applicable prize to an alternate winner by random drawing from among all remaining eligible entries. Prize will be shipped 1-2 weeks after the drawing of the sweepstakes.

Conditions:

  • All prizes are non-refundable and non-redeemable. No cash substitution will be available. Prizes may be substituted should they be unavailable due to circumstances beyond Pharmacy OneSource's control.
  • Winners are solely responsible for all federal, state, and local taxes that are applicable.
  • Pharmacy OneSource reserves the right to use contest winner's name, voice, photograph, and/or likeness for promotional/publicity purposes at any time without advance permission or knowledge required, and without additional compensation or limitations.
  • Winner is responsible for all other expenses except as listed under prizes in the contest rules. Pharmacy OneSource will not provide any money for any expenses incurred as a result of claiming or using the prize.
  • Due to unforeseen circumstances that can arise from time to time, the contest can be discontinued at any time during the promotional period.
  • Pharmacy OneSource reserves the right of final decision in all matters regarding the contest.
  • Rolex has not endorsed and is not affiliated with this giveaway. ROLEX is an official trademark of Rolex SA.

Limitations:

  • Pharmacy OneSource and affiliated companies, officers, directors, agents or employees are not responsible for any expenses the winner(s) incur due to illness, injury, or loss of life in claiming or using their prize.

Rules:

  • Any person(s) violating any of the above rules may be required to forfeit prize(s).
  • Rules may be modified at any time during the contest period with such changes announced and available on PharmacyOneSource.com.
